HOW TO COMPLETE
THE COURSE REQUEST SCAN SHEET
1. Submit the R710, format #9 to create course request scan sheets. The submission screen should look like this:

Format No: 9
School Year: Enter the Scheduling Year
Grade: 09 12
2. Have MDECA print your course request scan sheets.
3. Complete scan sheets
a. Enter course numbers as they are listed on CRSE. If there are no leading zeros on CRSE then don’t bubble in any leading zeros on the scan sheets. (EX: If CRSE has course #7400 then bubble in 7400 on the scan sheet. If CRSE has course # 0535 then bubble in 0535 on the scan sheet. Bubble in exactly what is on CRSE.) See Example attached.
b. The field “new grade level if different than above” is no longer applicable. Leave blank.
c. Use a #2 SOFT lead pencil – NEVER use ink.
d. Fill in bubbles completely.
e. Erase any stray marks from the sheets.
f. Collect scan sheets. Do not staple, fold or mutilate scan sheets in any way.
g. Scan course requests using format #09 and SCIN interface on SIS. SCIN can be run in the day queue. Follow the scanner instructions.
h. The scanned information will be placed in the REQU screen.
i. Check the B710 on the MGMT screen for errors that may have occurred during the scanning process.
