MEDI – STUDENT MEDICAL INFORMATION
MEDI screens are used to store, maintain, and update student medical information. This is an overview of SIS MEDI subscreens CO-Contact and HA-Health Alert. For more detailed medical information, use the SIS Medical Module.
MEDI subscreens CO-Contact and HA-Health Alert store information such as a medical alert, medical condition, doctor’s phone, parent’s phone, allergies, medications and brief medial history as well as a small amount of additional information. When the field "Medical Alert" is flagged as "Y" on student ID related screens, an "M" will be blinking above the student’s name indicating that there is a medical alert.
MEDI SCREEN INTERACTION
MEDI screen CO-Contact is a DISPLAY ONLY screen that displays emergency contact records and contact information from the following screens (Line # refers to line number on the MEDI subscreen CO screen sample):
Line 1-2 Records from CONT where the P113, Medi field, contains a Y
Line 3 PADR records where the “Display on MEDI” field contains a Y
Line 4 Parent/Guardian field from STUD
Student Contact codes must have "Y" in the Medi: field on parameter screen P113 to display on a student's medical record, MEDI subscreen CO.
P113 Student Contact Types Screen
This student's contacts AA-1st Emergency and EC-Emergency
Contact will be displayed on MEDI subscreen CO because they are coded "Y" in
Medi: field on parameter screen P113.
MEDI screen HA-Health Alert is used to record medical condition, allergies, medications and brief medial history. If the field "Medical Alert" is flagged "Y", then student ID related screens will display blinking "M" above the student’s name, indicating that there is a medical alert.

| Student ID | Enter the valid student ID
defined on the STUD screen. If you do
not know the student ID, press the <BACKSPACE > key and enter a portion
of the last name of the student you wish to locate. |
| Type | Enter HA for health alert. |
| Medical Alert | Enter "Y" if the
student has a medical condition. Enter "N" if the student does not
have a medical condition. If this field contains a "Y", then above
the student's ID, a blinking "M" will be displayed, on related
screens. |
| Condition | Enter up to 40 characters to define the condition of the student. |
| Allergies | Enter up to 2 lines of 67 characters each to define allergy information
about the student. |
| Other | Enter up to 11 lines of 67 characters each to define other medical information about the student. |
